How to apply and what will happen

Step 1. Request an application form by calling 020 8770 8286 or download a printable version (99 kb). This form refers to a FAQs sheet and charging policy. We apologise for the fact that these items are not yet ready. Please contact us on 020 8770 8286 if you require more information.

Step 2. The form should be completed by a therapist or medical professional who is working with the young person. This professional will need to be available for assessment appointments and prepared to take responsibility for ensuring that the equipment provided is suitable for the young person’s needs.

Step 3. Send the form to: The Project Administrator, MERU, 8 Damson Way, Orchard Hill, Carshalton, Surrey SM5 4NR. Your application will be discussed at our Project Referral Committee and one of the following will happen:

  • The project will be accepted and you will receive a letter explaining this. We will give your project a priority rating, primarily based on urgency of need. This helps us ensure that the most pressing cases are seen soonest. Once the project reaches the top of our waiting list, you will be contacted to arrange an assessment appointment.
  • We may ask you for more information to help us make a decision as to whether or not to accept the project. This may involve meeting the young person, their parent or carer and the referring medical professional, to help us understand what is needed.
  • We may inform you that we cannot help you on this occasion and will explain why. We will always endeavour to find you other possible sources of help.
