A few easy steps to apply to MERU for assistance
1. Check you meet our eligibility criteria – Click Here for information.
2. Request an application form by calling 01372 725203 or Download a printable version (pdf).
3. The form should be completed by a therapist or medical professional who is working with the young person. This professional will need to be available for assessment appointments and be prepared to take responsibility for ensuring that the equipment provided is suitable for the young person’s needs.
4. Send the form to The Project Administrator, Unit 2, Eclipse Estate, 30 West Hill, Epsom, Surrey KT19 8JD. It is advisable to phone to ensure it has been safely received.
Your application will be discussed at our Project Referral Committee and one of the following will then happen:
- The project will be accepted and you will receive a letter. Each project is given a priority score, primarily based on urgency of need. This ensures that the most pressing cases are seen soonest. Once the project reaches the top of our waiting list, we will arrange an assessment appointment.
- Alternatively we may ask you for more information to help us make a decision as to whether to accept the project. This may involve meeting the young person, their parent or carer and the referring medical professional, to help us understand what is needed.
- Or we may inform you that we cannot help you on this occasion and will explain why. We will always endeavour to find you other possible sources of help.
